The Michigan Department of Health and Human Services has specific eligibility requirements for both schools and students.
• Eligible schools must:
• Be closed or have reduced in-person learning for at least 5 consecutive days in the current school year
(beginning no earlier than September 9, 2020).
• Be a provider of the National School Lunch Program
PLEASE NOTE: Niles Community Schools did not qualify as an eligible school since the majority of our students received in-person instruction for the majority of each month.
Since this is the case all students who were virtual students these months and qualified for free/reduced meals can complete the Reconsideration form below.
The building your child is enrolled in can provide you with the necessary documentation that must be included with the P-EBT Reconsideration Form.
PARENT/GUARDIAN RESPONSIBILITIES: It is your responsibility to complete the P-EBT Reconsideration form, retrieve the necessary documentation from the building your child is enrolled at and mail the form to the address provided on the form.
